The Washington State Department of Health has set up certain guidelines for vaccinations required before students can attend school. In accordance with this, we require families to provide a valid Certificate of Immunization Status (CIS). Families that opt out of certain immunizations must provide a valid Certificate of Exemption (COE) signed by a healthcare provider.
Both of these certificates can be downloaded from our website:
- Certificate of Immunization Status (don't forget to sign!)
- Certificate of Exemption. Please note that if you choose to exempt your child from any vaccines, they will not be allowed to attend school in the event that there is an outbreak of any disease they are not vaccinated against.
These forms can be completed and faxed by your doctor's office at 206-364-0349, or can be turned into the school office. Don't forget the two parent signature fields on the Certificate of Immunization Status.
Parents of High School Students: There is a new requirement for the 2016-2017 school year. All 9th-12th grade students are required to have two (2) doses of the varicella vaccine. Please double-check your records and update your student's immunization records in the school office.
